Annual wellness & prevention
A yearly look ahead: what's going well, what to screen for, and a plan to stay independent and healthy.
Medicare Annual Wellness Visit
A health risk assessment, review of your medications and providers, cognitive screening, a fall-risk check, and a personalized prevention schedule. It's a planning visit, not a head-to-toe physical.
Preventive physicals
An adult physical with exam, vitals, and age-appropriate labs, for patients whose plans cover them, or as needed for work or travel forms.
Screenings & vaccines
We track which cancer screenings, bone-density tests, and vaccines are due, then arrange them or send referrals. Some vaccines are best given at your pharmacy, and we'll tell you which.
Health is more than lab values.
At home we can also see the things that matter for staying independent: lighting, stairs, nutrition, and how daily routines are going.

Questions
Is the Annual Wellness Visit free?
Medicare Part B generally covers the Annual Wellness Visit without cost-sharing once every 12 months. If other problems are addressed during the same visit, those may be billed separately and may involve cost-sharing. We'll explain this up front.
Can the wellness visit be done by video?
Much of it can, though we recommend in-home visits when vitals, a fall-risk assessment, or lab work are needed.
Should I bring anything?
Your medications, a list of your other doctors, and any recent test results. The visit prep planner builds a checklist for you.
